Sometimes something gets checked in when it should not be. To deal with this, you can mark an Encounter (Soap) as invalid. To do this you Go to the patient history and find the encounter that you would like to invalidate. On the encounter listing there is a check mark in a circle. Click on this to invalidate the encounter.
If you make a mistake, you can see all invalid items in the patient history by selecting the "Include invalid notes" filter.
